Volvo has another one of these out there. Remember the self-braking car that ass-ended a semi (I think)?

Yes at a press event. it did work the first time and Volvo says it was human error in setting it up that caused the problem.

That's a no-shit-sherlock statement. Considering it's a human-built system in a human-built machine, any failure is human error.
